The Transition Action Networks are forums for education and training providers, parents, carers and families of people with a disability, industry, government and community sector representatives with a youth focus to foster partnerships and create innovative initiatives to maximise the positive outcomes of young people with a disability (10 to 25 years) in their transition from school to further education, training and subsequent employment.
